Nosferatu Deformities

For myself, repugnant x 3 doesn't automatically equal instant Masquerade breach. It means your character is not normal (or attractive) by society's standards. Since vampires don't exist, that word isn't going to be the first thing that comes into someone's mind when they see your character. Unless they're another form of supernatural creature, I suppose. The average mortal would be thinking probably one or more of the following:

Call 911!
Ewww! I hope that's not catching!
I'm glad I'm not you!

To the average mortal your character could look like one of the following:

Person in need of medical attention.
Person with a disease or condition of the skin or body.
Accident survivor (burns, knife attack, etc.)

Your character makes people uncomfortable. You're not walking around with a sign that says:
VAMPIRE! RIGHT HERE! TAKE A GOOD LOOK!

The more subtle you can make your character's deformity, the creepier.

Some ideas: jaundice, bad teeth, different colored eyes (like orange or yellow), visible veins, the classic "look" from the movie, misplaced eye, deformed nose.
